People who agree with this admission agreement of our branch indicates the intention of enrollment by signing this agreement.

Our branch gives permission to people who have signed this agreement.

 

 

Article 1 Admission Qualification

 

You agree to this admission agreement  and have to satisfy all the

 following requirements 1. to 3.

 

１．One moderate politely

２．A person who isn`t apart of a group against public order and morality.

      The one that is unrelated to a group against public order and morality.

３．A person that is unrelated to an antisocial group.

       If you satisfy all the requirement, and you are a member of another school, Please consult previously with the branch             manager about admission permission.

Article 2 Admission Application

 

The person that approved of this admission must perform the following application procedure　1.～４.

 

１．If under 18 years old submit a parental permission form.

２．Fill out the payment transfer paper of the bank, and, please submit it.

  　 (The transfer fee is the responsibility of the student.)

３．If you want to pay by cash , You can put cash in an envelope and pay when you are first admitted.

４．Please take out sports insurance or accident insurance and so on.

　 　Please take into account Article 4 “１．Our branch office does not take responsibility for the compensation

　　 from an injury or the death, Because we instruct to cater to ability and the physical condition of our students.”

 

Article 3 Admission Day

The date our branch director gave permission to admission.

Article 4 About the Training

 

１．Our branch office does not take responsibility for the compensation from an injury or the death, Because we instruct to

  　 　cater to ability and the physical condition of our students.

２．If you disobey our instructions you may be expelled. After that our branch will take into account the Article 7 withdrawal

       admission. We will decide the consequences of our students.

３．When a students is ill-conditioned or in a dangerous situation accompanied danger, our instructor instruct go to cancel            the training.

４．We do not take responsibility any damages or theft in the bicycle parking lot or the training room.

５．The student participates in the clothes proper.

       (Karate wear, Judo wear possible)

６．When you pass a promotion kyu-ranking and dan-ranking examination, your recommendation of the promotion is sent            to the Okinawan headquarters director. You will be certified by the Okinawan headquarters director of myobukan.

 

Article 5  Monthly fee (Starts in April,2023)

　・Elementary students : 2,000yen

　・Junior high students and above : 3,000yen

    ・Adults : 4,000yen

  ※For students who don’t have accident insurance, Please get sports safety insurance instead.

 

   (Sports safety insurance fee)

    ・Elementary students : 800yen/year

　・Junior high students and above : 1,850yen/year

　・Adults：1,850yen/year

  

Article 6 Training day and time

 

  ・PM1:00-4:00 in Saturday and AM9:30-12:00 in Sunday

Article 7. Withdrawal Admission

 

When a student performs the following acts, we file for withdrawal without the agreement of the student and will not refund fees.

 

１．When a student doesn’t pay a fee consecutively for three months without contacting our branch.

２．When a student violates our agreement.

３．When a student doesn’t not satisfy requirements of article 1 admission qualification. And a disciple had falsehood in

　　Article 1  admission qualification.

４．When a student disobeys, the instructions and disturb the rules of our branch.

５．When a student performs acts of sabotage, an annoying act, contrary to public order and morals for the instructor of our         branch, a branch manager, a deputy branch manager and other students.

６．When a student belongs to a school other than our branch office without permission

７．When a student participates in the games of other schools without permission

８．When students have a match against each other without permission.

９．When a student gets the personal information of other students and solicits you for various activities.

Article 8 Personal Information

 

１．We can not give any personal information of a student to others.

２．We e-mail guidance, for example a lesson or event information to a student and may contact them by mail, telephone              or Fax.

 

 

Article 9 Copyright

 

Information about our branch’s HP in place.

 

１．A copy right and the other rights of our HP appertain to  our branch.

２．Our branch prohibits the use of unauthorized use,  a reproduction, a transfer, sale, a rental, publication, broadcast,

   　or any publication to the net.

 

 

Article 10 Revision of the Admission Agreement

 

Our branch may revise the admission agreement without a notice about the revision details.

Article 11 Observance of Your Manner

 

Obey the manners of the Budo.

When student don’t obey , we will give those students our attention each time.

 

 

Others

 

１．When people enter or leave the training room, they have to bow

２．Each person is responsible for their valuables.

３．Take off all accessories and jewelry.

４．Act according to the instructions of the instructors.

５．Cutting your nails (the people who can’t cut your nails, please  notify us)

６．When you are not feeling well , please let on instructor know as soon a possible.

７．Take  all your garbage home with you.

８．Prepare a drink for hydration.

９．No whispering or using cell phone in the training room.
